2-2.2 Registration Policies
Residents and property owners may obtain a card by completing
a registration form in person and presenting a photo ID,
proof of residence, and a social security number, current
driver’s license number, or current Department of Motor
Vehicles Identification Card number. A driver’s license
may provide all of the above. Proof of residency must include
physical street address. A post office box or business address
is not acceptable. Proof of residency may also be confirmed
by one of the following:
· Utility bill, not more than two months old, issued
to applicant (examples include gas, electric, sewer, water,
cable or phone bill). (Cellular phone and pager bills are
not accepted).
· Current home, automobile, or life insurance bill
(cards or policies are not accepted).
· Certified copy of school records/transcript from
a school in which applicant is currently enrolled, issued
by a U.S. state, jurisdiction or territory OR a Virginia
Department of Education Certificate of Enrollment form (a
report card is not acceptable).
· County decal receipts.
· Deed, mortgage, monthly mortgage statement or residential
rental/lease agreement.
· Postmarked U.S. mail with forwarding address label
or change of address confirmation form from the U.S. Postal
Service.
· Payroll check stub issued by an employer within
two months of the application.
· Original monthly bank statement not more than two
months old issued by a bank.
· Cancelled check (not more than two months old)
with both name and address imprinted.
Non-resident Virginia citizens may obtain a card after presenting
current photo ID with address and identification number as
above. Non-residents temporarily residing in Amherst County
must provide their local and permanent addresses.
By signing the registration card, the registrant agrees
to comply with the rules of the library and to pay whatever
fines or fees may be incurred for damages or losses of library
materials through the use of their card. A parent or guardian
must sign the registration form for persons under 18 years
of age. In so doing, the parent accepts responsibility for
the child's use of the library and agrees to pay fines and/or
fees incurred for misuse of library materials.
